occurred that brought you to file this complaint/incident
report

contact information for any witnesses
It would also be helpful for
you to include:

The driver’s
name
-
The driver’s
Taxicab Authority Permit number
-
Physical
description of the driver and any other participants in the
incident
NOTE: Notary information has
been placed at the bottom of this form to facilitate it being
used as an affidavit. If the Taxicab Authority finds it
necessary to use this information as part of an investigation,
this form may be returned to you for signature and notary
stamp.
